Dear Colleagues,

I would like to thank the membership for giving me the opportunity to serve as President of the Georgia Society of Oral and Maxillofacial Surgeons. It is an honor and a privilege and I hope to continue the successes of my predecessors.

This has been an eventful spring and summer. Natural and man made disasters dominated the news including earthquakes, oil spills hurricanes ash clouds, and floods. Health care reform, legislation on illegal immigration, troop withdrawal from Iraq, record unemployment and the November elections also captured our attention.

We as a specialty have had a busy year. The third molar study has been continued and a multidisciplinary conference on third molars will be held on October 19, 2010. The science to be put forward at this event will help to counter the unfounded statements being made about the lack of benefit in removing asymptomatic thirds molars. Providing anesthesia for our patients has continued to demand attention The American Society of Anesthesiologists plans to release an advisory regarding the education and training requirements for deep sedation for non anesthesiologists. Our current training should meet the necessary training requirements put forth in this advisory, however continued vigilance is necessary. The AMA Scope of Practice document also commanded the attention of our association leadership who are committed to pursue all avenues to have this document withdrawn by the AMA.

The Annual Meeting in Chicago was received enthusiastically by the members in attendance. The premeeting symposium on Maxillofacial Oncology and Reconstructive Surgery was very well attended. The scientific events were of high quality and the social events provided fun for all.

GSOMS has held two excellent meetings this year. Plans for the 2011 Spring meeting have been made.  The theme is the current management of temporomandibular disorders.
Issues that will be addressed by the GSOMS leadership this year include denial by Medicaid for impacted third molar removal, botox injections by oral and maxillofacial surgeons, maintenance of the office anesthesia evaluation program, oral and maxillofacial surgeons participation in the Georgia Trauma Network and specialty certification in oral and maxillofacial surgery in Georgia.

Georgia Board of Dentistry Report

The revision of the anesthesia rules outlining the educational qualifications and experience required for sedation permits and general anesthesia permits were adopted by the Georgia Board of Dentistry in the spring of 2010.  The complete revised rules can be found on the Secretary of State website – sos.georgia.gov/plb under the Dental Board page.  The revised rules are in line with the new ADA guidelines.  Permits are issued for moderate enteral sedation, moderate parenteral sedation, and deep sedation/general anesthesia.  The facility requirements for moderate enteral and/or parenteral sedation have been upgraded to require ACLS or PALS for the permit holder, AED, emergency drugs and IV fluids, and emergency airway management.  We are presently reviewing another revision to require that all support personnel involved with direct patient care be certified in BLS CPR and that a minimum of two support personnel be present during the administration of any moderate sedation or general anesthesia case.  As oral surgeons, we have always adhered to this AAOMS guideline but this requirement has not previously been included in the rules pertaining to all levels of permits.

Several of our GSOMS members provide their time and expertise to serve as anesthesia consultants and perform our anesthesia evaluations for new applicants for sedation and general anesthesia permits.  This is certainly a sacrifice and takes away from your productivity in the office, and the reimbursement has been less than nominal.  I have lobbied the Board and would like to inform those of you that serve as consultants that the previous flat rate of $100 has been replaced.  The new rate is $50 per hour up to a maximum of six hours/$300 per evaluation to include your travel time both ways plus the evaluation time.  Still not a reflection of how valuable you are to the Board, but a small improvement.  Please note that the Board is always interested in considering additional anesthesia consultants.  Don’t hesitate to apply.

Many of you have anesthesia permits for more than one location and know that the Board has in recent years required an on-site facility evaluation for each additional location or change in a location.  The Board has always had the option of waiving the facility evaluation and accepting a certification from you that any additional location or change in location meets the facility requirements as outlined in the rules.   At the discretion of the Board and in lieu of an on-site evaluation, certification only may now be accepted.  A certification form has been developed that has a check off list of equipment and supplies that are required and that were present at your original location during your initial evaluation.  Of course, the equipment and supplies must be stationary at each location.  Remember, you must have a permit for each and every location at which you administer anesthesia.  Anyone that has a pending facility only evaluation can contact the Board to request the certification form.  This will save you and our consultants time and money.

I am pleased to inform you of the recent report from AAOMS showing that Georgia has a high compliance rate with the self regulation and society governed anesthesia review initiative from AAOMS. But we still have work to do. Several years ago the AAOMS decided it to be in the best vested interest of the specialty of Oral and Maxillofacial Surgery to continue to monitor our own anesthesia practices and make sure that we are up to date and compliant with our education and with our equipment. Oral and Maxillofacial surgeons provide safe anesthesia to our patients and have a tested track record of being responsible and pragmatic practitioners.  However, recent economic trends and a desire by others to regulate the practice of anesthesia have seen our specialty under assault from legislators and other practitioners.  The AAOMS office anesthesia program and review system will help us to make sure that we are providing the most up to date and safe services for our patients that we can.

Georgia currently has the 7th highest percentage of complete evaluations in the country and the 2nd highest in our own district.  Juxtaposed to states with comparable numbers of  surgeons we are a close second to Ohio with 189 surgeons to our 170 surgeons.  These positions are not to be lauded but to say that we are on the right track and need to have 100% compliance by 2011.  If you or a colleague has not completed your examination-please lets “Finish the Drill” and get Georgia into compliance.  If you have any questions about your exam or whether AAOMS has your certificate ready please don’t hesitate to contact myself or Ms. Leslie Boulter at the GSOMS Administrative Office at 770.271.0453.

MCG OMFS Program

The MCG OMFS program currently has 7 residents and 2 interns. The senior and chief resident is Dr. Daniel Scott from South Georgia, and the third year resident is Dr. Abtin Etezadi from Atlanta, both are MCG dental undergrad alums.  There are two second year residents, Dr. Patrick Walker from Savannah, also a MCG alum, and Dr. Hany Eman from Egypt who has completed a Masters in Oral Surgery.  First year residents, Dr. Jeremy Hixson and Dr. William Klein both completed OMFS internships at Parkland Hospital in Texas and Dover, Delaware, respectively.  The current two interns are Dr. Mathew Kikuchi who graduated dental school in Las Vegas Nevada and Dr. Carlos Pierla who is from Venezuela.

This year’s faculty and residents have been maintained a busy clinical load, as well as, lecturing and publishing.  Dr. Mark Stevens, Professor and Chairman, completed his Bone Harvest Atlas with Dr. Robert Marx in February.  He has also had several articles published, one in the Journal of Oral Implantology and one in the New Journal of Craniofacial Trauma with Dr. Martin Salqueiro titled, “The use of Pre Bent Plates in Mandibular Reconstruction”.  In the past year, Dr. Stevens has lectured not only nationally, but also internationally in South America and Korea.  This year, Dr. Stevens will be presenting on The Management of Angle and Symphysis Fractures at the AAOMS meeting in Chicago, IL.  He and Dr. Scott, senior resident, will also be presenting an abstract on “Bisphosphonate Osteonecrosis in an Animal Model”.  Dr. Etezadi, along with Dr. Shahrohk Bagheri, will also have a poster on microscopic repair of long buccal nerves at the AAOMS meeting.

Dr. Henry “Butch” Ferguson, Vice Chair and Program Director, presented a lecture (for the 2nd year in a row) at the University Of Georgia’s Homecoming. The presentation was sponsored by the Georgia Academy of General Dentistry (GAGD).  Dr. Ferguson also has an article published in The Journal of American Dental Association (JADA) titled, “Dental Care for Patients with Heart Failure: An Update” and a chapter on Pre-Prosthetic Surgical Considerations in the 6th edition of the Textbook of Complete Dentures.

Dr. Solon Kao, Assistant Professor, a graduate of our OMS program at MCG, has been on the faculty since 2005.  He recently traveled to Washington DC to participate in the AAOMS’ 10th Annual Day on the Hill program.

Dr. Martin Salgueiro, Assistant Professor, is also a graduate of our MCG OMS program and our newest faculty member beginning in 2009.  Currently, his main responsibility is to provide clinical coverage for the undergraduate students and OMS residents and interns.

The Department also held our first annual Academic Day Continuing Education in June 2012. Dr. Myron Tucker was the guest lecturer who spoke on Segmental Orthognathic Surgery.  Dr. Tucker, along with the OMS residents, presented on unique cases and cutting edge surgical techniques.  We would like to invite the GSOMS to our next ”Academic  Day” in June 2011.  We truly appreciate the GSOMS support of our residents and the MCG program.
